TET2-VAF-Adjusted CMML Risk Calculator

Refines the CMML-specific Prognostic Scoring System – Molecular (CPSS-Mol), the Groupe Français des Myélodysplasies (GFM) model and the Mayo Molecular Model (MMM) by integrating TET2 variant allele frequency (VAF) as a continuous prognostic variable.

How it works

  1. VAF adjustment is read from the per-integer-VAF lookup (Table S9) derived in the UK discovery cohort using restricted cubic spline (RCS) Cox regression with the empirical first quartile of TET2 VAF in mutated patients (39%) as the reference (HR = 1.0).
  2. Wild-type patients receive a fixed adjustment derived from a binary Cox model comparing TET2-WT to mutated patients in the reference zone (VAF 33–45%): CPSS-Mol +1.78; GFM +2.68; MMM +2.96.
  3. Adjusted score = original score + adjustment.
  4. Risk category is assigned by likelihood-ratio-optimised (LR-opt) cut-points derived in the discovery cohort, preserving the parental number of categories.
  5. Expected outcomes are the Kaplan–Meier estimates for the corresponding category in the discovery cohort (Table S10).
